Purpose:The licensed master social worker will assist in the facilitation of care across the continuum for selected patient care needs with a focus on the following: evaluation of psychosocial needs, crisis intervention, development and implementation of appropriate, timely plans of care to include discharge planning; ability to collaborate with case manager, physicians, staff, family and community based resources to enhance and encourage optimal patient outcomes; evaluation of plan of care with appropriate changes, based on plan outcomes.Qualifications:Master's degree in Social WorkUp-to-date and unencumbered license with the Louisiana State Board of Social Work ExaminersCompetent in the following:* Effective communication* Problem-solving* Stress management* Computer skills* Attention to detail* Demonstrates flexibility* Ability to express ideas both verbally and in writing
