Developmental Pathways (DP) is seeking an Intake Case Manager to support the Systems Navigation (SN) Team.

You will conduct in-home assessments, coordinate access to Medicaid Waiver and state services, and engage families to build trusted partnerships.

The role is hybrid with remote work options, requiring travel within the Colorado catchment (Arapahoe, Douglas, Elbert).
